Aetna Utilizing Medical Home for its Medicare Advantage Populations

In efforts to improve quality of care and lower health care costs, Aetna and the Connecticut State Medical Society Independent Practice Association (CSMS-IPA) announced on March 13, 2012 in the New York Times that they are designing a program based on the collaborative care model for its Medicare Advantage members.  Nurse case managers and physicians will collaborate in order to improve quality of services and provide better care management to this population. Quality and efficiency measures in various clinical areas will be used to monitor improvements in adherence to best practices and treatment plans.Having more than 500 physicians affiliated with CSMS-IPA in the designated areas, this program will be far-reaching with future plans to expand the program to physicians in additional counties.  Having already experienced positive results for similar collaborative care programs in Connecticut, Florida and Ohio as well as PCMH programs in Connecticut and New Jersey, Aetna  is undertaking the development of products and services that support value-driven, patient-centered care.
