CMD Goals & Deliverables

The CMD will convene and report out at general meetings of the PCPCC progress towards achieving the following objectives/goals:


Primary Objectives: 

  • Describe the activities of sites developing and implementing the patient-centered medical home (PCMH) model;
  • Share information and best practices about PCMH activities within a collaborative community; and
  • Serve as the connector to technical, quality improvement, and education resources to facilitate ongoing demonstrations.

Specific Ongoing Goals:

  • Track and disseminate information on developing and ongoing test projects of the PCMH throughout the country, with a primary focus on multi-payer/multi-stakeholder projects;
  • Gather and present information to support the development and implementation of the PCMH model, including best practices, critical success factors, and challenges (e.g., antitrust concerns, reimbursement strategies, practice transformation approaches, technology, team work and training programs, etc.); and
  • Collect and share information on:  the evaluation design of developing and ongoing PCMH test projects; the measures being used in these projects, including both process and outcome metrics; and the types of experience and satisfaction surveys being employed, including those of patients, physicians, and other stakeholders.  

 

Fiscal Year 2009-2010 Deliverables
 
Below is a list of deliverables that has been updated since the release of the 2010 Strategic Plan to reflect changes desired by the CMD leadership. 
 
Update 1 and 2 reflect CMD’s completion of both deliverables. In reference to Update 3, CMD plans to develop a survey tool to obtain feedback. The survey will be placed on their website section and at the end of the Center call agenda. Furthermore, the survey will include questions regarding topics that call participants would like CMD to engage. For Update 4, CMD plans to update the pilot section of their website section with projects like those listed in the section. In developing such a pilot section, CMD aims to make it more searchable i.e. a search on access only projects. Update 5 concerns planning learning collaborative breakouts in upcoming PCPCC conferences. Update 6  is a new deliverable concerning a possible Care Coordination Taskforce. Details are in the box below. Concerning Update 7, CMD plans to author a Transformation Guide similar to the Consumer Guide in format, i.e., it would include an environmental scan, etc. Finally, Update 8 concerns the recent decision to associate previously independent Taskforces with Centers. The Payment Reform Taskforce should be integrated into CMD.

*Update 6: Care Coordination Taskforce - In a recent proposal to the Board, CMD has requested forming a Care Coordination Taskforce. Such a taskforce would consider questions on how practices are handling issues such as: transitions in care, specialists, home health, ACO, hospitals, etc.
