The Centers for Medicare and Medicaid Services (CMS) will be hosting an Open Door Forum (ODF), Tuesday October 28th from 2:00 PM to 4:00 PM EST, on the Center's Medicare Medical Home Demonstration (MMHD) project.  The conference call is intended to inform physican practices and physicians about the design of the Medicare Medical Home Demonstration project.
