Maine Patient Centered Medical Home Pilot

Attached, please find materials on the work being done by the Maine Patient Centered Medical Home (PCMH).  The Maine PCMH focuses on keeping the consumer "front and center" in the medical home process.  They aim at engaging the patient by including their voice in three areas: Involving Consumers in Planning and Governance, Conducting PCMH Consumer/Patient Focus Groups, and Involving Patients in PCMH Pilot Practice Redesign Efforts.
