The Patient-Centered Primary Care Collaborative (PCPCC) is pleased to provide you with the attached patient education brochure and checklist on the Patient-Centered Medical Home.  These resources are intended to help health care professionals communicate with patients about the Patient-Centered Medical Home approach. The brochure is intended to stay within a health care professional's office and the checklist is intended for a patient to keep.
