This is a reminder that our next PCPCC/PCMH briefing will be held on Tuesday, April 13th, from 11:00 AM to 12:00 PM EST.  The Executive Director of the Patient-Centered Primary Care Collaborative (PCPCC), Edwina Rogers, will speak over the phone for any organization or individual who wishes to learn more about the Patient-Centered Medical Home (PCMH) model and our Collaborative.  This briefing is free of charge.
