 We are a diverse group of individuals representing stakeholders including clinicians, consumers, insurers, researchers and businesses on the front lines of health care delivery who believe that integrated patient-centered care is necessary for the success of the Patient-Centered Medical Home (PCMH). We believe that a “whole person orientation” and team approach to care would be incomplete without incorporation of behavioral health care, including prevention, tobacco cessation, substance abuse and mental health services.
