Snapshot of the Evidence:


In a first-of-its-kind study, the Primary Care Collaborative's 2019 evidence report examines states’ primary care spending patterns, including spending across payer types, and considers the implications of these results for select patient outcomes.

These results follow on the heels of the 2018 evidence report, which examined the interaction between Patient-Centered Medical Homes (PCMHs) and Accountable Care Organizations (ACOs).

The 2017 evidence report aggregated outcomes from 30 peer-reviewed studies, state government program evaluations, independent evaluations of federal initiatives and industry reports to evaluate the impact of the PCHM model on cost and utilization measures:
