Sarah (Sally) Bleeks is a managing director in the Office of Clinical Affairs at the Blue Cross and Blue Shield Association.In this capacity she provides leadership for the patient centered medical home initiative and for health care disparities.

Since joining the Association in 1989, she has been involved in a variety of consulting and development activities including nurse training, utilization management, managed care benchmarking, national account network evaluations, medical policy and business development.  She was a member of the team that evaluated a $14 billion pharmacy benefit management contract for the Federal Employee Health Benefit Program.  She also led the development and maintenance of the Blue Distinction Centers for Bariatric Surgery and provided oversight for the Blue Distinction Centers for Transplant.

Her baccalaureate degree is from the University of Iowa, and her master of Science degree is from the University of California.  She also holds a Certificate of Professional Development from the Wharton Executive Education Program of the University of Pennsylvania.
