Purpose of the project is to accelerate the adoption of the PCMH model of care in support of a larger community based health improvement initiative.  The Program will monitor the impact on outcomes, quality, and cost for members in self-funded plans. The Project will begin with an education and awareness campaign and expand to assisting physicians with certification process.  The Project will assist major community medical provider groups in the development of accountable care organizations (ACO).
