Patient Centered Medical Home:  Stakeholder's Working Group Meeting

Come join hundreds of people who believe that solutions to the health care system's toughest problems CAN be found by talking and working together. Some of the topics that will be discussed include PCMH practice transformation and developing the patient centered medical home team. Speakers will address the current policy landscape, explore the implications for the patient centered medical home and discuss implementation challenges at the local level.

The PCPCC's Center leadership will host speakers on topics of interest to membership of each of the Centers, such as benefit plan redesign, use of health information technology to support the medical home, pilot project challenges and solutions and public sector initiatives. Attend this event and hear viewpoints and ideas from those engaged in the national discussion on health reform.
