The National Academy for State Health Policy (NASHP) and the Patient Centered Primary Care Collaborative (PCPCC) have partnered to work on an initiative to advance patient centered medical homes in Medicaid and SCHIP beneficiaries.
This one-year project is supported by The Commonwealth Fund and is based on the joint principles of the Patient Centered Medical Home Model (PCMH) developed by the AAP, AAFP, ACP, and AOA, among others. These principles include: personal physician; physician directed medical practice; whole person orientation; coordinated and/or integrated care; quality and safety; enhanced access to care; and payment that reflects value. In recent months, several states have considered or passed new legislation or regulations to promote the PCMH.

NASHP and the PCPCC, with the advice of a group of state policy makers and other experts, are planning a working meeting – the Medical Homes Summit – to support developing state initiatives in medical homes. We plan to bring to Washington, D.C. teams from up to ten states that are developing or have committed to developing initiatives. These teams will meet with their peers who have already implemented strategies to support medical homes, as well as nationally recognized experts. Together, this group will examine promising practices, brainstorm new approaches, and begin to apply the lessons learned to participating states’ plans.
NASHP and the PCPCC have released an Invitation for Applications to invite states to apply to participate in the Summit.
