    Focus on Patient Partnerships
    Patients and families should play a key role in helping clinicians, administrators, health plans and employers transform the US health system through strong primary care by way of patient-centered medical homes (PCMH). The goal of the PCMH is to actively involve patients and their families as partners in their care, as well as in designing, implementing, and evaluating improvements to health care delivery. This panel will describe how partnerships with patients and families can lead to improvements in health care coordination, quality and safety.

    Stories from the field - Creating a PCMH
    All across the country, clinicians are forging new partnerships to support their transformation to patient-centered medical homes. Clinicians are finding new ways to engage with patients that include necessary changes in practice leadership, implementation of team-based care, and integration of new technologies to name a few. This panel will demonstrate that there are a multitude of strategies that can be employed on the path toward becoming a PCMH and speakers will detail how they are working toward that transformation in their practice and community.

    Day 2 Welcome and Collaborative Award Ceremony - Sponsored by URAC
    The first PCPCC Annual Award will be given posthumously to Dr. Barbara Starfield, MD, MPH, in recognition of her research on the benefits of primary care in reducing inequities in health, and her enduring commitment to assuring all individuals receive comprehensive, coordinated, person-centered care in partnership with a trusted primary care physician. Starfield was professor of health policy and management at the Johns Hopkins Bloomberg School of Public Health when she died last year at the age of 78. She was an accomplished researcher and champion of the need for a strong primary care system in the U.S. and worldwide, and her work led to the development of important methodological tools.

    Patients often struggle to navigate the health care system when they are required to seek care beyond their primary care clinician's practice. Through improvements in information sharing and better care coordination, the PCMH plays a critical role in ensuring patients have access to high quality, comprehensive care that is delivered efficiently and in partnership with the patient and his/her family.     Employer/Payer Dialogue
    Many employers and payers have embraced the PCMH as a way to improve health for their employees and increase efficiencies in the health system. Several employers are partnering with payers to link value-based benefit design with reimbursement models that will support PCMH care in their communities. This panel will host a conversation with employers and payers who are developing innovative strategies to help their beneficiaries access care through PCMH’s.
