Patient Centered Medical Home: Opportunities & Solutions

Registration is Open

Come join hundreds of people who believe that solutions to the health care system's toughest problems CAN be found by talking and working together. Some of the topics that will be discussed include PCMH practice transformation and developing the patient centered medical home team. Speakers will address the current policy landscape, explore the implications for the patient centered medical home and discuss implementation challenges at the local level.

The PCPCC's Center leadership will host speakers on topics of interest to membership of each of the Centers, such as benefit plan redesign, use of health information technology to support the medical home, pilot project challenges and solutions and public sector initiatives. Attend this event and hear viewpoints and ideas from those engaged in the national discussion on health reform. If you are dedicated to solving problems through honest talk, quality thinking and collaborative action, this gathering is for you!

The Patient Centered Primary Care Collaborative is a coalition of major employers, consumer groups, patient quality organizations, health plans, labor unions, hospitals, clinicians and many others who have joined together to develop and advance the patient centered medical home. The Collaborative has over 400 members and believes that, if implemented, the patient centered medical home will improve the health of patients and the viability of the health care delivery system.
