This is not a pilot, but the  report of an Alaska family practivce residency that has achieved NCQA PCMH Level 3 recognition.  The Alaska Family Practice Residency began a PCMH transformation in April 0f 2009.  In November 2010, the AFPR applied to NCQA for Level 1 recognition and received it in February 2011.  AFPR updated their PCMH application for Level 3 recogntion and submitted it to NCQA in November 2011. Level 3 recognition was awarded in February 2012.
