New initiative focuses on patient and medical home model for Kansas

The Kansas Academy of Family Physicians announces a new initiative focusing on the Patient Centered Medical Home (PCMH) model of health care delivery. The Kansas Patient Centered Medical Home Initiative was launched in January 2011 with grant funding from the Kansas Health Foundation, Sunflower Foundation, and United Methodist Health Ministry Fund.  The three year initiative will include enhanced physician engagement, public awareness and a pilot project for eight practices.  

A coalition of primary care physicians groups developed the Kansas PCMH Initiative proposal.  The organizations involved in the Kansas Primary Care Physician Coalition are:

·         Kansas Academy of Family Physicians (KAFP)

·         Kansas Association of Osteopathic Physicians (KAOM)

·         Kansas Chapter of the American Academy of Pediatrics (KAAP)

·         Kansas Chapter of the American College of Physicians  (KACP)

·         Kansas Medical Society (KMS)

The Kansas PCMH Initiative will be led by the KAFP with assistance from coalition members and TransforMED, an organization specializing in practice redesign, which is affiliated with the American Academy of Family Physicians. Formation of a grant Advisory Council including representatives from the health professions involved in the PCMH team is included in the initiative.
