Led by the Georgia Academy of Family Physicians, the Physicians Institute for Excellence and TransforMED, a cohort of 24 small sized (5 or fewer physicians) primary care practices and 3 family medicine residency programs, are undergoing an 18 month learning collaborative whichwill ultimately end with the practices applying for NCQA recognition as a PCMH.  Ongoing discussions with both public and private payors surrounding this project seeking enhanced payment for all NCQA recognized practices in Georgia.
