Created in 2005 as a not for profit subsidiary of the American Academy of Family Physicians, TransforMED has been a leading authority on transforming primary care practices into Patient-Centered Medical Homes. TransforMED strongly believes that a stronger future for primary care practices, health systems, and the health of the patients they serve can be achieved, in large part, through transformative practice redesign. TransforMED’s experienced staff is comprised of experts with a wide competence in areas of clinical and business improvement, as well change management. TransforMED also collaborates with outside researchers, consultants and a diverse group of specialty, industry and policy stakeholders. In the current health care environment, affecting sustainable changes in delivery models and stakeholder behavior requires more than subject area competence, it requires the fundamental trust from participating providers and their staff.
