Dr. Dennis Weaver is President and CEO of EastPoint Health and Project Manager for the Adirondack Health Institute.
Cynthia Nassivera-Cordes is Vice-President of Medical Support for the Hudson Headwaters Health Network.
Overview: Learn about the latest information technology advancements and other strategic developments for the Adirondack Region Medical Home Pilot, a joint initiative of medical providers and public and private insurers to transform heath care delivery in a rural, upstate New York Region. The goals of this pilot project are to improve care, expand access and contain costs using the patient-centered medical home model for health care services.
