Dear PCPCC Members,
Thanks to all of you who have already completed the survey, but we still need more! We would love for those working in primary care practices to take a few moments to tell us a little bit about how your practice is integrating wellness and prevention. If you have contacts with those working in primary care, please forward the link to this survey to them. We rely on your input to deliver information that will be most beneficial to you in helping create patient centered medical homes!

Wellness and Prevention Survey
The PCPCC is seeking information on how primary care practices address wellness and prevention for patients and families who are part of a patient-centered medical home (PCMH). This is a 2-part survey. Part 1 will take approximately 10 minutes to complete and asks for responses to general inquires regarding the integration of wellness and preventive services into primary care practices. We will use the information collected through this survey to guide the development of a publication on Wellness and Prevention in the PCMH. You and/or a knowledgeable staff person from your practice can complete this part of the survey anonymously.
For Part 2 of the survey, we are seeking models for how to provide wellness and preventive services in a PCMH that we may share with others. We are seeking more detailed information about specific activities and strategies you have undertaken to address a particular element of wellness and prevention in your practice. If selected, and upon receiving express permission, your information will be shared with others in a future PCPCC publication on Wellness and Prevention. Only de-identified summary information will be reported. This portion of the survey will take about 15 minutes to complete. You DO NOT need to complete Part 2 of the survey if you do not wish to submit a model practice.
We would also like to solicit your assistance in reaching additional clinicians. Please forward this link through your network of organization and individual primary care clinicians so we can maximize our survey responses. Thank you for your support in these efforts.
